Form Builder
The RFG Contact Us fome builder provides the ability for an administrator to create custom forms. Form fields can be simple text or scrolling text boxes or they can have options represented by radio buttons or drop down list boxes. Once the form has been submitted by an end user, the form data gets tracked and organized under administration where it can be pushed through a workflow process until the request is completed.
As new forms are created, the CMS site page edit logic will detect the new form and allow it's selection for insertion on a page.

Form List |

From this form list page an administrator can add, edit or delete a form. |

Form Edit |
|
The form edit page sets up the construct for the form. From the form detail page the administrator can then add custom field labels (questions) and associate options to more complex questions. |

Form Questions |
|
Each form can have an unlimited number of optional form fields (questions). Each question produces a label on the form. The questions can be dragged and dropped to order the questions as you see fit. |

Question Edit |

Adding or Editing a question allows the user to type a label (question) and then select a question Type: text box, scrolling text box, date box, drop down list, check box, multiple check boxes, radio buttons, title/spacer. |

Question Options |

Once a question is assigned a type that requires options (radio button, drop down list, checkboxes, etc.), the administrator will then enter 1 or more options for tht field. |
